TransCare
Our goal is to improve the transitional care pathway from hospital to home by using technology assistance. Another goal is to reduce the rehospitalization rate aiming to establish a clinically validated value proposition.
TransCare is a project funded under the THCS programme in EU.

​We will study how the care pathways will need to be redesigned for allowing the integration and usage of the technology in different settings providing the necessary knowledge for care relocation from hospital to home (payment models, hospitals incentives).
We will enhance and scale already developed technology to a larger number of patients and consider the specific contexts of different healthcare systems in Europe as well as different types of multimorbidity patterns.
The partners in the TransCare project come from Italy, Norway and Romania, all having different transitional care pathways. In Italy and Romania, next of kin and family pay an important role. In Norway, municipalities undertake much of the responsibility in the care pathway.

The project is financed by the Italian Ministry of Health, the Research council of Norway and the Romania Research Council. In addition, the partners provide own financing.​​
