Work packages 

The TransCare project is composed by 5 work packages.

WP1 Project management

WP2 Transitional care innovative technological solution adaptation

WP1 is devoted to monitor the scientific and technical progress of the project; To establish an effective project management structure to plan, monitor and coordinate the project; To take care of the financial management and to perform risk management; To promote efficiency and quality of work in the project and provide administrative and scientific coordination; To manage ethical and social issues, gender aspects and intellectual property right (IPR); To report and maintain liaisons with the THCS CMU and to assure networking with other THCS projects.​​

WP2 will refine and improve the remote patient monitoring infrastructure and ML based post discharge assessment and follow-up. We will consider the local context of each trials site, roles and responsibilities, language, etc. The TransCare platform is based on intelligent personalization targeted towards patients and family members. Functionality will be adapted to enable remote interaction between patient and health personnel (e.g., questionnaires, support for self-treatment planning, dietary suggestions, chat etc).

WP2 is devoted to the development of technology foreseen for coaching and supporting the cognitive function of older adults with MCI. it encompasses the development and fine tuning og the technological services including robot-based programming, and their integration into a complete engAGE system prototype. This WP will be closely aligned to WP2 (co-creation) and WP3 (testing and evaluation) and will serve the commercialization and business requirements of WP4. Ensure that ethics by design is integrated in all tasks.

WP3 Transitional care knowledge building and care pathways design

WP3 will identify local challenges in transitional care. This information will be used to adapt technology, case pathways for understanding of current practice helping to identify where gaps in care occur. Ethical standards, in terms of fair treatment of participants, respect of human rights, accountability in research, data protection and security, anonymity, etc. will be taken under careful consideration to ensure compliance with European legislation & criteria.

WP4 Longitudinal pilots

In WP4, the complete methodology for the different longitudinal studies will be detailed, including definition of target populations, primary and secondary outcomes, quantitative and qualitative tools to be administered to primary, secondary, and tertiary users as well as description of technical components, data management of the collected information and intervention description. 210 participants will be included with the aim of testing the proposed solution for a meaningful period of at least 3 months. The measurements collected from the real-life trials will be elaborated to evaluate the qualitative and quantitative indicators and a complete impact assessment analysis will be carried out.

WP5 Dissemination and exploitation

WP5 is devoted To raise public awareness on the results and receive feedback through this website in addition to Facebook and LinkedIn posts; To issue reports, articles and other dissemination material and to participate in networking activities; To support exploitation of results, analysing the market characteristics and its potential; To define an appropriate exploitation strategy; Consider replicating solutions and lessons learnt from one pilot site to other pilot sites and identifying the replication strengths and weaknesses for other EU level contexts Ensure that Ethics by Context is integrated in all tasks. Ensure proper IPR and risk management

The Technical University of Cluj-Napoca (TUC), Romania, is the largest technical university in Transylvania and one of the best technical universities of Romania (it is ranked as an A-class (top) research university in Computer Science and Information Technology domain). The Distributed Systems Research Laboratory (DSRL – https://dsrl.eu/), part of the Computer Science Department, Faculty of Automation and Computer Science, has extensive experience in the next research areas: ambient intelligence, big data analytics, multidisciplinary optimization, blockchain, modelling and simulation. DSRL coordinates and is involved in several research projects out of which, the following AAL projects are relevant for engAGE: H2HCare (co-ordinator, https://h2hcare-aal.eu/), ReMember-Me (scientific and technical responsible – http://www.aal-europe.eu/projects/remember-me/), ReMIND (https://www.aalremind.eu/), DIET4Elders, Elders-UP! and MedGUIDE.

HINS, Romania

HINS is one of the most important tertiary medical centres in Romania dedicated to the vast field of CVD including prevention, diagnosis and interventional or surgical treatment. HINS team research activity carried out until the present has focused on studying the risk factors for acute coronary syndrome and secondary prevention in the case of patients suffering from cardiovascular diseases. The team has published more than 100 research papers in their research domain.

Karde AS, Norway

Karde is a multidisciplinary SME that offers consultancy and development services in accessibility of ICT, assistive technologies and e-learning, with emphasis on users with cognitive decline or intellectual disability. Karde has worked with these areas for over 10 years. Karde has coordinated and participated in several national and international projects to benefit the target groups. In the ongoing H2020-project REMIND Karde is working with reminding technologies for PwDs. In national projects, Karde has developed several e-learning systems for people with intellectual disabilities. Karde has participated in several AAL-projects: Mylife (coordinator, AAL-2011), T&Tnet (AAL-2011), PIA (coordinator, AAL-2012), Home4Dem (AAL-2015), MedGUIDE (AAL-2016), and GUIDed (AAL-2018). As part of the projects, Karde has engaged end-users in testing and pilots in a co-creation setting. Through AAL-project Mylife, Karde developed the memory assistant MEMAS, for PwDs. Karde established a company and got investors to bring MEMAS to the Norwegian market. Karde has also excellent knowledge of the use of assistive technologies in real home settings.

Farsund municipality has for several years been active in both international and national projects to improve the health service in the municipality. All 25 municipalities (above 300.000 inhabitants) in the Agder region have focused on the use of e-health and welfare technology solutions to meet society’s future challenges, including a growing elderly population and lack of health and social care resources. The region has participated and been a central contributor in several projects (2 EU and 4 national) within the field of e-health and welfare technology and other fields of study.

IRCCS INRCA Istituto Nazionale di Riposo e Cura per Anziani, Italy

INRCA is the leading Italian public Institute (with 1076 employees) in gerontology and geriatrics, devoted to improve the quality of life of older persons, through different centres in Italy, comprising geriatric hospitals, an Alzheimer cure centre, and a scientific-technological area. The objectives of the Institute are focused on successful ageing and the promotion of health of the older person. Currently, there are four lines of research: (1) Biogerontology: cellular determinants, molecular and genetic aging, and age-associated diseases; (2) Prevention and treatment of frailty: management of geriatric diseases and syndromes; (3) Aging and Medicines and (4) Multidimensional assessment and continuity of care. Great emphasis is given to technological innovation, promoting telemedicine and acceptance of technology by older people. INRCA has established a large network of longstanding contacts with older people’s and users’ organisations, both at Italian as well as at European level, through involvement in multiple AAL projects as coordinator or participant since the first call, as follows: Resilien-T, Guardian, Living well with Anne, Eware, Happy Ageing, ChefMyself, HAAL, MagicTable, Home4Dem. Dr. Roberta Bevilacqua is the responsible contact for IRCCS INRCA.
