Agder conference 19.3.2026

Agder is the most Southern region in Norway where Farsund municipality is located.

Camilla Gabrielsen from Farsund municipality presnted the TransCare project and Digital Home Monitoring.

Digital Home Monitoringis a health service where patients with chronic conditions receive close follow-up by health professionals in their own homes via technology, often tablets and medical measuring equipment. The patient measures values ​​(e.g., blood pressure, oxygen saturation) themselves. The data is then transmitted to the health service, who contacts them in the event of a deterioration.

Tellu presented their TelluCare platform which has been further developed and being used in the TransCare project:

TransCare Joins Forces with TechSocialcare in Cluj-Napoca, 18.2.2026

TransCare’s coordinator, Prof. Ionut Anghel of Technical Univertity in Cluj-Napoca, participated in a joint meeting with the TechSocialcare project — an Interreg Europe initiative dedicated to promoting technical standards for assistive technology in European social care services. The event, held in Cluj-Napoca, Romania, was organized by Napoca Porolissum in their capacity as a TechSocialcare partner.

During the meeting, TransCare’s coordinator presented the project and engaged in meaningful knowledge exchange with attending stakeholders, covering various aspects of project implementation. The encounter proved particularly valuable given the strong overlap between both projects’ networks. The TechSocialcare consortium includes key actors highly relevant to TransCare’s work, such as the NSRF Ministry of Health (Greece), the Ministry of Health of Canton Sarajevo (Bosnia and Herzegovina), the Municipality of Agnone (Italy), or Vidzeme Planning Region (Latvia). A highlight of the day was the discussion from the co-creation workshop, where participants explored shared priorities including ethics, interoperability, standardisation, usability, and digital literacy.

Norwegian Research Council, gathering for THCS-projects with Norwegian partners, 12.12.2025

The Norwegian Research Council organised a gathering 12th December 2025 at their premises. The purpose of the gathering was to create a meeting place for the projects, with a focus on the exchange of experiences. All of the eight THCS-projects with Norwegian partners were invited. The projects were presented and discussed. Very different projects, all of them.

Terje Grimstad from Karde gave the presentation of TransCare which is about rehabilitation at home after discharge from hospital. Karde provides the communication platform Memas. Other Norwegian partners in TransCare are Tellu who provides surveillance of discharged patients at home by the use of medical devices and Farsund municipalty which is reponsible for recruiting patients and following them up.

HINS gave a presentation of TransCare to general practioners in Cluj-Napoca.

Lister municipalities, 13.2.2025

Farsund municipality gave a presentation of TransCare to Lister municipalities. Lister consists of 6 municipalities in the Agder region which collaborates to debvelop health care services. Sirdal, mentioned above, is one of the municipalities in Lister.

The meeting addressed issues like ethical standards, data management, and the preparation of longitudinal pilots. Also, dissemination, exploitation and communication strategies, innovation, replication and risk management were discussed.

Project meetings

Project meeting in Farsund, 17-18 September 2025

The meeting was held in the beautiful town of Farsund in the southern Norway.

In the meeting, updates were provided on project management, technological developments and the design of patient centred care pathways. Special focus was on ethical standards, data management, and the preparation of longitudinal pilots that will test innovative solutions in real world heath care environments.

The second day of the meeting was dedicated to dissemination, exploitation and communication strategies. Innovation, replication and risk management were addressed to ensure that the results of TransCare can be adopted widely and generate lasting impact for health systems and society.

Project meeting in Ancona, 12-13 March 2025

The meeting was held in the beautiful premises of INRCA.

The purpose of the meeting was to present status on technological developments, finalise patient care pathways, plan the trials and start work on exploitation.


Kick-off in Cluj-Napoca, 30-31 May 2024

The purpose of the meeting was to bring partners together and discuss and decide on the action plans of the project. All WPs were adressed. Technologies to be used in the project, care pathways from Italy, Norway and Romania were presented and discussed. Ethical issues were addressed and initial planning of trials performed.
